Blood pressure is force, measured twice in each heartbeat
Blood pressure describes the force of blood against arterial walls. The first number, systolic pressure, reflects pressure when the heart contracts. The second, diastolic pressure, reflects pressure while the heart relaxes between beats.
The unit is millimetres of mercury, written mmHg. A reading such as 128/78 therefore contains two different measurements, not one score.
Why one reading is not the whole diagnosis
Blood pressure changes with activity, stress, pain, caffeine, temperature and measurement technique. A single elevated reading can therefore be misleading.
WHO defines hypertension as blood pressure of 140/90 mmHg or higher and states that diagnosis is made when elevated readings are confirmed on two different days. Other guidelines use different thresholds. In the United States, for example, hypertension is commonly defined from 130/80 mmHg upward.
These differences are not proof that one organisation has discovered a different human physiology. They reflect different approaches to risk thresholds, diagnosis and treatment.
High blood pressure is usually silent
Most people with hypertension feel no symptoms. That makes routine measurement essential. Waiting for headaches or dizziness is unreliable because significant hypertension can exist for years without obvious warning.
WHO estimated that about 1.4 billion adults aged 30–79 had hypertension in 2024, and about 44% were unaware of it.
What persistent pressure does to the body
Arteries are living tissues, not rigid pipes. Chronic high pressure contributes to vascular damage and accelerates cardiovascular disease. It increases the risk of heart attack, stroke, heart failure and kidney disease.
The harm comes from exposure over time. That is why an otherwise healthy-feeling person can benefit substantially from treatment.
Measurement technique matters
A cuff that is too small, talking during measurement, unsupported feet, a full bladder or measuring immediately after activity can distort readings. Home monitoring can be useful, especially when clinicians suspect white-coat hypertension or want to assess control outside the clinic.
Validated upper-arm devices are generally preferred. A clinician should interpret repeated readings in context rather than relying on an isolated number.
What raises blood pressure
Age and genetics matter, and so do modifiable factors. WHO identifies high salt intake, physical inactivity, excess body weight and excessive alcohol use among important risks. Diabetes and kidney disease can also increase risk.
Air pollution has also emerged as an environmental contributor. This makes hypertension both an individual and population-health problem.
Salt matters because sodium affects fluid and vascular regulation
WHO recommends reducing sodium intake to less than 2 grams per day, roughly 5 grams of salt. In many diets, most sodium comes not from the salt shaker but from processed foods, sauces, breads, packaged snacks and restaurant meals.
Reducing sodium can lower blood pressure, particularly in people who are salt-sensitive.
Lifestyle helps, but medication is often necessary
Physical activity, a healthy diet, lower sodium intake, weight reduction when appropriate, avoiding tobacco and limiting alcohol can lower blood pressure. Yet many people still need medication.
Common drug classes include ACE inhibitors, angiotensin receptor blockers, calcium-channel blockers and diuretics. The correct choice depends on age, kidney function, pregnancy potential, other conditions and treatment goals.
Treatment targets are individual
WHO notes that for many people the general goal is below 140/90, while lower targets such as below 130/80 may be recommended for people with cardiovascular disease, diabetes, chronic kidney disease or high cardiovascular risk.
That is why a target should be discussed with a clinician rather than copied from an internet chart.
When high blood pressure becomes urgent
Very high blood pressure can sometimes accompany severe symptoms such as chest pain, shortness of breath, confusion, neurological symptoms or visual changes. WHO advises urgent care when very high pressure—often around 180/120 or higher—is accompanied by concerning symptoms.
A number alone should not be interpreted without context, but severe readings with symptoms require prompt medical attention.
Blood pressure naturally rises and falls during the day
Blood pressure is dynamic. It rises during exercise and emotional stress and falls during rest and sleep. A single number therefore has meaning only in context.
Clinicians care about the usual level of pressure over time. Repeated measurements, and sometimes home or ambulatory monitoring, help distinguish sustained hypertension from temporary elevation.
White-coat and masked hypertension show why location matters
Some people have higher readings in a clinic than at home, often called white-coat hypertension. Others show the reverse pattern: apparently acceptable clinic readings but elevated pressure outside the clinic, sometimes called masked hypertension.
These patterns are one reason home monitoring or 24-hour ambulatory blood-pressure monitoring can be useful. Diagnosis should not be based on casual self-measurement alone, but out-of-office readings can provide important context.
How to measure at home more reliably
A home reading is more useful when technique is consistent. The person should sit quietly for several minutes, keep the back supported and feet on the floor, place the cuff on a bare upper arm at heart level, and avoid talking during the measurement. Caffeine, exercise and smoking shortly beforehand can affect readings.
Using a validated device with the correct cuff size matters. A cuff that is too small can overestimate pressure. Recording several readings over multiple days is generally more informative than repeatedly checking during a moment of anxiety.
Why the systolic number often becomes more important with age
Arteries tend to stiffen with age. As a result, systolic pressure often rises even when diastolic pressure does not rise to the same extent. Isolated systolic hypertension is therefore common in older adults.
That pattern is not harmless simply because the lower number looks normal. Treatment decisions still depend on overall risk, frailty, kidney function and tolerance of therapy.
Blood pressure treatment is about preventing future events
A person may take an antihypertensive medicine and feel exactly the same because hypertension often had no symptoms to begin with. The benefit is largely preventive: reducing the probability of stroke, heart attack, heart failure and kidney damage over time.
This can make adherence psychologically difficult. The treatment does not necessarily produce an immediate sensation of improvement. Its value is the reduction of future risk.
Low blood pressure is a different problem
Not every lower number is automatically better. Some people naturally have low blood pressure without symptoms, while others develop dizziness, fainting or falls when pressure is too low, especially after medication changes or dehydration.
Treatment targets should therefore be individualised. The goal is not to push blood pressure as low as possible regardless of symptoms; it is to reduce cardiovascular risk safely.
Blood pressure should be interpreted alongside total cardiovascular risk
The same blood-pressure reading can lead to different treatment decisions depending on age, diabetes, kidney disease, previous cardiovascular events and other risks. A clinician may recommend medication sooner when the overall probability of heart attack or stroke is high.
This total-risk approach helps explain why treatment cannot be reduced to a universal internet threshold. The measurement is important, but the person behind the measurement is equally important.
Pregnancy can reveal blood-pressure risk
High blood pressure can also arise during pregnancy, including conditions such as gestational hypertension and preeclampsia. These require obstetric assessment because they can threaten both maternal and fetal health. A history of preeclampsia also carries information about later cardiovascular risk, so it should remain part of a person’s long-term medical history rather than being forgotten after delivery.
Consistency matters more than one perfect reading
The most useful pattern is repeated, correctly measured readings over time.
The important habit is knowing, not guessing
Blood pressure is powerful partly because it is measurable and treatable. It does not require sophisticated imaging to identify risk.
The central public-health problem is therefore not that hypertension is mysterious. It is that many people do not know they have it, and many who know are not adequately controlled.
Regular measurement, correct interpretation and sustained treatment can prevent complications long before symptoms appear.
